Premise Alert Program

The Premise Alert Program allows residents with special needs, disabilities, or medical conditions to voluntarily provide important information to the Plainfield Fire Protection District before an emergency happens. When our crews respond to your address, this information appears on their dispatch screen — helping them assist you more effectively from the moment they arrive.

The program is open to anyone living within the PFPD service area. Registration is voluntary and free. You control what information is shared, and you can update or cancel your registration at any time.

Who Should Register

This program may benefit you

The examples below are common reasons residents register, but the program is not limited to these situations. If there is anything you'd like first responders to know before they arrive at your home, you're encouraged to register.

Autism or Developmental Disabilities

A family member who is nonverbal, may not respond to verbal commands, or may become agitated by unfamiliar people, noise, or lights.

Alzheimer's or Dementia

A resident who may be confused, unable to follow directions, prone to wandering, or unaware of an emergency situation.

Medical Conditions or Equipment

Conditions such as epilepsy, oxygen dependence, dialysis, insulin pumps, or other medical devices that responders should know about.

Vision or Hearing Impairment

A resident who is deaf, hard of hearing, blind, or has low vision and may not hear alarms, announcements, or verbal instructions.

Mobility Limitations

A resident who uses a wheelchair, walker, or is bedridden and may need assistance evacuating or accessing areas of the home.

Anything Else

Any information you believe would help first responders serve you or a member of your household more safely and effectively.

Register

Complete the form below or submit a printed enrollment form with your information.

We Review

PFPD receives your submission and verifies the information provided.

CAD Entry

Your information is entered into the dispatch system (CAD) through WESCOM, PFPD's dispatch agency.

Crews Informed

When 911 is called from your address, responders see your premise alert on their dispatch screen before they arrive.

Your Information

Privacy & managing your registration

The information you provide is entered into the Computer-Aided Dispatch (CAD) system and is accessible to PFPD personnel and WESCOM dispatchers responding to calls at your address. The program is entirely voluntary — you choose what information to share, and your submission does not guarantee a specific type of response. It provides responders with context that helps them better assist you.

If your situation changes, you can update or cancel your premise alert at any time by calling (815) 436-5335 or emailing plfd_fire@plainfieldfpd.org.

This program is established under the Illinois Premise Alert Program Act (Public Act 96-0788), which allows citizens with special needs or disabilities, their families, or caregivers to provide important information to police, fire, and EMS personnel to more effectively respond to emergency situations.

Before you submit

Use the form below to register for the Premise Alert Program. Include the address, the name of the person the alert is for, and a description of the information you'd like first responders to be aware of. Be as specific as you feel comfortable — the more detail you provide, the better prepared our crews will be.

By submitting this form, you acknowledge the following: The information provided is intended to offer guidance and information to responders in assisting those people with special needs or disabilities in the performance of their duties. Presenting this information will not entitle or result in any form of preferential treatment. This information will be updated every two (2) years. A notification, whether public or private, will be made prior to that 2-year deadline. It is the responsibility of the submitter to notify the Plainfield Fire Protection District, in writing, of any changes to this information as soon as those changes are known. The information entered into the Premise Alert Program (PAP) database shall remain confidential. This information will be relayed to responding public safety personnel via computer or any means available. The submitter verifies that the above-listed person has a physical or mental impairment, has or is at increased risk for a chronic physical, developmental, behavioral, or emotional condition and also requires health and related services of a type or amount beyond generally required by individuals. The submitter is the named individual, a family member, friend, caregiver, or medical professional familiar with the individual. By submitting, you certify that you have read and understand this notice in its entirety and hereby give permission to the Plainfield Fire Protection District to enter this information into the Premise Alert Program (PAP) database.
